How to make a request for information in an accessible / alternative format
Blackpool CCG is committed to embedding the new Accessible Information Standard from NHS England into its work. To do this the CCG ensures its information is fair and accessible to all sections of our local community meaning the public and staff can request reasonable adjustments such as information into other formats.
To request information or any of our key documents in an alternative format such as braille, larger print, audio or other format please email: firstname.lastname@example.org quoting your address, telephone number along with the title and date of the publication plus the format you require. In addition you may require additional support to contact CCG eg a British Sign Language interpreter to support you or you may need language support where your first spoken language is not English.
